  1. Go to www.tactitans.org
  2. Click Sign in (upper right) and enter your credentials
  3. Go to the left navigation and click Org Resources > Resources, then you'll see the forms

 

FINANCE FORMS

(1) Suspension / Medical Suspension / Contract Buyout Form

If your swimmer is in the DEVELOPMENT TRACK, or if you are a MASTERS SWIMMER use this form when:

  • There is a MEDICAL ISSUE keeping you/your swimmer out of the water for >21 days.
    (Doctor's note required.)

  • You wish to SUSPEND billing temporarily for a non-medical reason OR
    If your swimmer is leaving the program (cancel entirely)

If your swimmer is the CHALLENGE or ELITE TRACK, use this form if your swimmer wants to leave the program entirely. This initiates a "Contract Buyout".

 

(2) Reactivation Form - complete this form when swimmer is ready to return after a suspension

(3) Swimmer Movement Form - complete this form when swimmer is moving from one group to another

(4) Swim Academy Cancellation Form - ONLY for swimmers in the Learn-to-Swim / Lessons program
